We work closely with your Physician and referring agency by providing all necessary documentation.
Once you have decided to pursue independence in driving, call us to schedule your appointment for your driving assessment. Prior to this assessment you will need the following:
- A doctor’s prescription is required to complete any assessment procedures or training. This prescription may say “Drivers Evaluation and Training”.
- A valid driver’s license or learner’s permit is required for all in vehicle assessments and training.
- A list of your current medications will also be helpful in completing paperwork.
If you plan to have a third party pay for this assessment, authorization must be received in our office prior to scheduling the appointment. Third party payers often include Vocational Rehabilitation, Independent Living Rehab Services, Veterans Administration, Workman’s Compensation and Family Trusts.
